Parent Data Confirmation and Medical Insurance
Be able to capture health insurance information and to populate the fields for Health Insurance Name and Student Medical Record Number/Policy Number in the Medical Insurance Page in Parent Data Confirmation.
In addition, Please add the following fields to the Medical Insurance Page: Medi-Cal True/False checkbox, Medi-Cal #, DHCS Confirmation Number, and Primary Care Physician, First and Last Name of the Policy Holder and if the insurance Is Primary or Secondary

5/12/25 Update:
We have been meeting with the Third-Party Administrator for CYBHI, Carelon Behavioral Health, in order to make sure that the next round of updates all conform to their established imports and processes. We are starting development on additional features for this project, which will be released as soon as they pass testing.
- Adding the ability to prompt and/or require parents to confirm their insurance information as part of Parent Data Confirmation.
- Adding additional fields to hold Subscriber information to the Insurance table.
- The fields to be added will be informed by the Insurance file requirements controlled by Carelon.
- Insitituting a drop-down menu with pre-poulated medical insurance carriers for easier data population and management.
- The list for Insurance type M (Medical will be pre-populated from the list provided by Carelon.
- There will be an option for "Other" that will allow a different insurance carrier to be entered into a free-form field.
- Other recognized carriers can be added by the district by utilizing the Update Code Table for this new field, for any of the Insurance Types
- Temporarily removing the Copy/Paste function in the Admin portal until guardrails can be enacted to limit the pasting of insurance records to only those linked by the Family Key. The ability for parents to copy/paste in the portal is already restricted to students they are directly linked to. This extra protection is needed for HIPPA compliance.
This space will be updated as these features come online.

Related to the new CYBHI Consent Form for California districts: https://www.dhcs.ca.gov/CYBHI/Pages/Fee-Schedule.aspx#:~:text=Overview%20of%20the%20CYBHI%20Fee,service%20plans%2C%20and%20disability%20insurers.
To initiate the process, the District requires proof of insurance (see attached). By submitting this form, you are providing consent for the school district to seek reimbursement for any physical or behavioral health services your child receives while at school. Please note, your insurance will not be billed or notified of services your student receives. There is no co-pay and no impact to your insurance coverage. Having insurance is simply the requirement to seek CYBHI funds
We also need Insurance/PDC to include:
Do you have insurance Y/N?
Insured P/G Name, DOB, and Employer name

Also, if possible, change the Company Name field to a drop down so that we can create a code table for site staff to choose from for uniformity.

Yes! This is an urgent need sooner rather than later. All of these ideas/fields mentioned please and an option to select "I do not have insurance at this time" if needed so the required page can be satisfied as answered and allow the parent to move forward with the enrollment or the data confirmation process.

This would be critical and has to be implemented for both PDC and Online Enrollment.
@Helen Encinas has stated very clearly what needs to happen for CYBHI grant, and according to my student services director, almost all districts in California will receive the grant this year. -

Customize the current insurance information pages to meet specific CYBHI requirements.Consider using built-in features or creating custom fields to capture the necessary information.

Due to the recent changes in medical billing for school districts "California Youth Behavioral Health Initiative," it is critical that the district be able to obtain medical insurance information from families that can be updated periodically. The ability to gather medical insurance information will afford schools the ability to bill for mental health services and expand support services to students. This is a California initiative, and you will likely hear from other school districts who will begin billing.

Can we also add the Subscriber number as well for the PDC. This is a definite for the current Insurance screen but if it is getting added to PDC this field is also important.

Please vote for this. We are trying to use Aeries for printing out billing so you don't have to use the NCR forms. Very important!!!!
