Auto populate last updated username and date timestamp info for Medical Log, Medical Notes, Medications, and Counseling
Auto populate the username and date time stamp to include modified by and modified when, in fields where the initials/or name need to be selected from a dropdown: Example, Medical log, Medical Notes, Medications, Counseling. Some areas like Attendance notes, Contacts and Discipline Records already have some of these features.

To facilitate faster data entry, our nurses would like the logged in user's employee ID number, start date and start time to auto-populate when visiting the Daily Medical Log to enter info. They would still like flexibility to change it but it would be extremely helpful if they were only required to modify when necessary.

It definitely seems like any fields where such notes and info are being entered that displaying the actual person who made the most recent change would be far preferably than letting users select their initials from a dropdown. If Person B changes an entry by Person A but doesn't change the initials it would be quite the headache to figure out who actually made the change.
