Home language survey - Language Options
Add the ability for parents to select more than one language for each question. Our EL coordinator stated CDE allows parents to enter more than one language for each question on the survey. Because this is not a current feature in Online Enrollment we continue to collect Home Language Survey in paper form.

The latest CDE-approved K012 Home Language Survey asks the parents to respond with one language for each question. See the form here: https://www.cde.ca.gov/ta/cr/elforms.asp
Therefore, we are declining this request.
