“I hereby certify that the information contained on this record is accurate and correct, that the assignments listed combined with the weekly lesson plan represent XX worth of instructional minutes on each day listed, and that daily participation, included absences was verified for each student in my class.”

Allow modification of verbiage to match district procedure on the Weekly Engagement Report that is verifying the weekly instructional minutes a teacher has completed
