Course History Institutions - Stop Alert for duplicate CDS Numbers

Course History Institutions
When entering a new Institution, if the CDS number already exists in Aeries, it should prompt a flag or stop notification. We've noticed that we have duplicate records.

    Also, adding a 'status' field (active/inactive) to the CHI table would help to clean up duplicate entries without changing student records.
